Case Management, Homemaker & Respite Services

Seniors interested in accessing any Frail and Elder services must request an appointment through the Frail and Elder Program Manager. Appointments generally are conducted at the senior’s home with whatever family the senior would like to be present. An assessment will be conducted to determine if the senior meets the program qualifications to receive services. Services available are listed below.

Non-Medical Case Management

Services include needs assessments, coordinating medical services, assisting with Medicare and Social Security issues, helping with prescription plans, benefits counseling, and guidance in accessing resources. Provides home visits and wellness planning.

Non-Medical Homemaker Services

Services include 1.5-hour visits a week with duties such as light housekeeping in main living areas, laundry, assistance planning and preparing meals, and shopping for groceries and other items.

Durable Medical Equipment Loan Program

Short-term, emergency assistance with medical equipment is available to frail, homebound elders in need of equipment such as walkers, canes, shower chairs, and toilet risers. Availability of equipment is limited and distribution is determined by need.

Non-Medical Caregiver Support

Caregiver Support services offer non-paid family caregivers who reside in the same household information about available services, distribution of educational material, and assistance in accessing available services to support their role as caregiver. Individual Supportive Counseling offers caregivers the ability to talk about their concerns and options relating to their caregiver role, providing resource materials and information to guide them in making decisions and identifying support.

Non-Medical Respite Services

In-home respite is offered to full-time non-paid family caregivers of frail homebound elders age 60 and older. Priority placement is offered to elders who have been diagnosed with Alzheimer’s or related dementia. 1.5 hours of respite time is offered to each eligible participant once a week.
