What is the Sandoval County Health Care Assistance Program?
The Sandoval County Health Care Assistance Program (HCAP) is a program that provides health care services to low-income residents of Sandoval County and can show proof that they have resided in the County for 90 consecutive days. The person may qualify for health care services provided through HCAP if they are at or below 185% of the current Federal Poverty Guidelines.
The health care services available to eligible persons are as follows:
- Ambulance
- Inpatient Hospital Services – No Emergency Room Visits
- Outpatient Behavioral Health Services
- Outpatient Primary Care Services (includes audiology)
- Senior Safety Essential, such as pain relief/balance support footwear and shower grab bars
- Pharmacy – Prescriptions and Medications per Medicaid formulary. Excluded are all Schedule 2 medications and special $5.00 prescriptions
- Cremations
Individuals who meet all eligibility criteria are eligible for the fiscal year which is July 1 through June 30. Individuals must apply each fiscal year to determine their eligibility for HCAP. All payments for services covered under HCAP are paid to the provider not the individual. HCAP pays only a portion of the cost, not the billed amount and may change the amount paid from one fiscal year to the next based on funding. Payments are never made to the person only to the service provider.

If you meet the HCAP Guidelines and qualify for the HCAP you are eligible to apply for assistance.
How does an individual apply for the HCAP?
The first step is to review the HCAP Checklist and answer questions one through four, if the answers determine eligibility proceed to the HCAP Application.
Fill out the application, attach required documents and contact a Provider to schedule an appointment: If you are not able to schedule an appointment with a Provider call the Health Care Assistance Program.
Application – English / Spanish
If an applicant is living with a relative or an advocate you are required to fill out the Verification Residency Letter.
- Verification Letter – This form is only used if an applicant is living with a relative, friend or advocate. English / Spanish
- Verification of Income – This form is only used if an applicant is working for cash or if a relative, friend or advocate is assisting the applicant financially. English / Spanish
