How to Apply for HCAP

The first step is to review the HCAP Checklist and answer questions one through four, if the answers determine eligibility proceed to the HCAP Application.

Fill out the application, attach required documents and contact a Provider to schedule an appointment: If you are not able to schedule an appointment with a Provider call the Health Care Assistance Program.

• Application – English / Spanish

If an applicant is living with a relative or an advocate you are required to fill out the Verification Residency Letter.

• Verification Letter – This form is only used if an applicant is living with a relative, friend or advocate.

• Verification of Income – This form is only used if an applicant is working for cash or if a relative, friend or advocate is assisting the applicant financially.
